Perinatal Infections

GBS
Prevention: Intrapartum Penicillin G or Clindamycin/Erythromycin
  1. All women with positive GBS urine culture
  2. Previous neonatal sepsis
  3. Trimester vaginal culture positive
  4. Preterm gestation
  5. Membranes rupture > 18 h
  6. Maternal fever
Toxoplasmosis
  • Chorioretinitis
  • Intracranial calcification
  • Symetrical IUGR
  • Hepatosplenomegaly
  • Thrombocytopenia
  • Primary infection cause problems
Treatment:
  1. Pyrimethamine and sulfadiazine
  2. Spiramycin to prevent vertical transmission
Varicella
  • Microphthamia
  • Extremity hypoplasia
  • Zigzag rashes
  • Greatest risk if rashes occur 6 days antepartum or 2 days postpartum
Prevention: VZIG, attenuated VZ
Treatment: Acyclovir

Rubella
  • Congenital deafness
  • Congenital cataracts
  • Congenital heart disease
  • Primary infection causes problems
Prevention: all pregnant women should be screened for IgG. Active vaccination after delivery

CMV
  • Nonimmune hydrops
  • Symmetric IUGR
  • Periventricular calcification
  • Congenital deafness
Treatment: Ganciclovir

HSV
  • If genital herpes presents, C-section
  • Acyclovir
